where in template html to insert 3rd column ?

From a brief look:

....<p>Copyright {year} - {homePageLink (siteName)}</p>
</td>
</tr>
</table>
</td>

add this:
<td>
{calendar}
<br clear="all">
your stuff
</td>

end your added html

</tr>
</table>
</body>
</html>

You specify where the calender goes - by adding the {calendar} macro to your
template.
