installing the survey's plug-in

Greetings all,

I find the docs on installing the survey's plug-in a bit confusing.

The web page docs say "Open surveys.root" I'm assuming this is from the
server menu, though I can't find the surveys.root in the list.

I have quit and restarted, but can't seem to get to step one.

Frontier 8.0.5 on Mac OS 10.1.4

Any tips? Thanks in advance.
