New Message: email subject and from address

We have set up a Manila site with email notification to a list of email addresses when discussion messages and stories are created. The setting for "send notification from" is set to "Instructional Technology Committee <kenneth.klemow at wilkes.edu>".

This used to appear as the return address. Recently, it has begun to be just "kenneth.klemow at wilkes.edu". Also, the subject is now blank. When you look in the message the From and Subject are listed there. How can we get the return address and subject back to the way it used to be?
