It can work with or without email confirmation, though for public sites I would recommend having both enabled to help filter out any spam bot requests.

Basically, after someone signs up with or without email confirmation, the user will then be instructed to fill out a form with a short message on why they want to sign up.

The managing editors of the site will then be notified through email that there's a pending member and you can approve/reject them from the new Pending Members Admin page.
