Hello again,

It appears as though we've solved the problem, with a hint from Lawrence
Lee, who recommended the following post regarding changing passwords:

This line from the post rang a bell: "This isn't a perfect solution as it
causes two sets of password entries to appear on the site signup form."

We took a look at our custom prefs, and it turns out that the two lines of
code suggested in the post had been added, resulting in the problems I
described in my earlier messages.  Deleting the two lines of code solved the
issue.
