Hello Bill,

One thing to try, re-create the user and block them from the website.

A guess as to what is going on. The Surveys plugin was the first plugin created. At that time members couldn't be deleted from Manila websites. Manila now allows members to be deleted but the plugin hasn't been updated in ages.
