New Message: Editors Only menu is not visible

I'm having a problem with multiple sites on my server. I'm logged in to the site and I'm the managing editor, but the Editors Only menu is not visible. When I go to /login, I get the message "You are already logged in as xxx" and then the Editors Only menu appears and things work fine after that. This happens for other editors also, but in some cases, going to the login page does not resolve the problem and they are not able to see the Editors Only menu at all. We've tried it on OS X and Windows XP using Safari, IE, and Firefox with similar results. For me, it has been like this for six months, but yesterday I found out that others had the same issue. Any ideas?
