The Radio Community Server issue has been fixed in the upgrader.

You could try retrying the upgrade and choose Main->Jump and enter:

manilaSuite.accessControl

to verify that table is there, which is new to 9.5.

If that table exists, you might also close all open browsers after upgrading to verify you aren't seeing a cached page.
