Steve,

Distorted backgrounds and letters are generally regarded as the poorest type of CAPTCHA to use as they can be farmed out to humans to solve.

Implicit CAPTCHAS are considered the best image based kind and have a better flow to the task at hand but are not so good for the visually impaired.

Text CAPTCHAS like the one I experimented with are good for both sighted and visually impaired but are somewhat more onerous because they break up the flow of the task at hand.
