I also had a problem creating new users.

The problem came when my cable company changed my ip numbers, and I  
made many changes, but I forgot to change the relay exceptions in my  
mail server. ... The mail server would not relay the email from  
Manila, so Manila would not create the new user.

Once I changed the relay paramaters on the mail server, Manila could  
send mail, and new users were created.
